Osborn Family Health Center serves all patients regardless of ability to pay. We offer a Sliding Fee Discount Program to eligible patients for services offered based upon family size and annual household income.
Osborn Family Health Center accepts several major health insurance plans and participates with Medicaid Managed Care Plans. Bring your insurance cards with you to your appointment.

CHARGES FOR HEALTH CARE SERVICES
All individuals that choose to receive health care services at Osborn Family Health Center are required to first register as patients and renew as patients annually.
During the registration and annual renewal process, patients are interviewed to determine method of payment and eligibility for various benefit health plans and programs, including eligibility for a Sliding Fee Scale discount. In accordance with the Federal Poverty Guidelines, Osborn Family Health Center’s sliding fee scale is based upon family size and income (see Sliding Fee Schedule on the BELOW). Osborn Family Health Center does not discriminate against any individual receiving health services because of their inability to pay.
