New Patients

Osborn Family Health Center serves all patients regardless of ability to pay. We offer a Sliding Fee Discount Program  to eligible patients for services  offered based upon family size and annual household income. If you or your family member has never been to Osborn Family Health Center, the first step is to call to schedule an appointment. All individuals that choose to receive health care services at Osborn Family Health Center are required to first register as patients and renew as patients annually. 50% of our staff are bilingual. We also provide a number of translated forms and handouts in other languages.  Let a staff member know if you need an interpreter or help reading a form.

Making an Appointment

  • Arrive on time for your appointment.  Call to notify us if you are going to be late.
  • If you are unable to keep your appointment, please call (856-757-3700) to cancel or reschedule the appointment.  This will allow us to see another patient in your place.
  • Most appointments are scheduled within 1-2 weeks of your call, so please call ahead for the earliest possible appointment. If the date of your choice is not available, ask to be placed on our call list.



Osborn Family Center is on the NJ Transit Authority bus route. Free parking is available for Osborn Patients. Ask for a parking voucher when leaving.
